When can I register?

Our first registration day is fast approaching and we’d like to answer some burning questions. If we didn’t answer one of your questions here, please email us, tweet us, or send us a private message on Facebook. When can I register? We’re holding our first registration day on Saturday, April 4th from 9:30 a.m. to 1 p.m. Where can I register? Registration is taking place at our new office located at 1637 Woodroffe Avenue. Take the elevator or stairs to the 3rd floor. Our office is in Suite 301. What do I have to bring with me? When you get here, we’ll introduce ourselves and give you some additional forms to fill out. Depending on how busy it is, we’ll give you a number to make sure that everyone is served on a first come, first served basis. If you want to become a patient, here’s what you need to do: Join our waitlist so that we can email you as soon as the registration days are finalized. Follow us on Twitter or ‘Like’ us on Facebook for updates. Complete a New Patient Form for each patient you’d like to register. If you have a preference, don’t forget to circle the name of the doctor you’d like to be assigned to (upper right corner of the New Patient Form). Bring the completed form and a valid Ontario Health Card on the day of registration. If you already have a family doctor or liveĀ in Quebec, registration days will be held later in the summer.
