How Do I Become a Patient?

How Do I Become a Patient?

In April, two of our family doctors are taking new patients and we hope you’re one of them.

Registration days for patients without family doctors or whose doctors are retiring this year are scheduled for early to mid-April.

If you want to become a patient, here’s what you need to do:

  • Join our waitlist so that we can email you as soon as the registration days are finalized.
  • Follow us on Twitter or ‘Like’ us on Facebook for updates.
  • Complete a New Patient Form for each patient you’d like to register.
  • If you have a preference, don’t forget to circle the name of the doctor you’d like to be assigned to (upper right corner of the New Patient Form).
  • Bring the completed form and a valid Ontario Health Card on the day of registration.

If you already have a family doctor or live in Quebec, registration days will be held later in the summer.

We’re thrilled that you’re considering joining the Mirus Medical team. We’re not just doctors and patients, we’re a team. Together, we will develop a successful care plan for you and your family.

We look forward to meeting you soon!

Dr. Hamilton & Dr. Mo

  1. Hello,
I noticed your outside signage regarding accepting new patients….I have not had a regular family doctor in over 10 years, so hopefully you have space for one more patient.
    Looking forward to your response
    Thank you

    • Jeff,

We are still accepting patients, with priority given to those like you who don’t currently have family doctor.

  3. Our family physician has moved from Centrepointe, Nepean to Orleans. My daughter, her 6 year old son and myself leave in Centrepointe. It is difficult to manage/schedule appointments with the new location of the clinic being so far. We would like to register as new patients with Dr Hamilton or Dr Mo for care. Do you have any space available.

    Regards.

    • Monique,

We do have some space still available for patients. The best way to roster is to stop by in person.

  4. Hi
Our Doctor of 40 years is retiring in April. Does Dr. Hamilton still have space for 2 more. We are both in our 60’s with no major health concerns and live in Bridlewood Kanata. I got her name off the board at Dr. Thornes office @ Broadview medical.

    Thank-you

    • Hello,

Thank you for the interest in our clinic. Unfortunately, Dr. Hamilton is no longer accepting patients. The clinic does maintain a waiting list for interested patients as we are currently recruiting new physicians to join our team. One physician just started and is taking patients from the waiting list. If you would like to be added to our waiting list please contact the clinic directly at 613-369-5060 or e-mail hello@mirusmedical.com.

      Thank you,
