When can I register?
We’re holding our first registration day on Saturday, April 4th from 9:30 a.m. to 1 p.m.
Where can I register?
Registration is taking place at our new office located at 1637 Woodroffe Avenue. Take the elevator or stairs to the 3rd floor. Our office is in Suite 301.
What do I have to bring with me?
Don’t forget to bring a valid Ontario Health Card and a completed New Patient Form for each patient you would like to register.
What if my child is over the age of 16?
- Your teenager must be present to fill out their own form.
What if my child is under the age of 16?
- You can add your child onto the forms you will fill out on Saturday.
What can I expect on Saturday?
When you get here, we’ll introduce ourselves and give you some additional forms to fill out. Depending on how busy it is, we’ll give you a number to make sure that everyone is served on a first come, first served basis. The registration process shouldn’t take more than 30 minutes of your time.
Can I still register if I already have a family doctor?
You sure can! All we ask is that you call the Ministry of Health at 1-888-376-5197 to “de-roster” with your current family physician before Saturday.
I can’t make it this Saturday. Are any other registration scheduled?
As of today, this is the only planned registration day. That might change and we’d like to keep you in the loop. Please join our waitlist or send us an email if you already joined and we’ll contact you when additional dates are added for registration.
